High-carbohydrate, high-fibre diets for diabetics.

چکیده
The concept of a more liberal attitude towards carbohydrate consumption in diabetics is almost as old as the discovery of insulin. Indeed, some of the most elegant studies in this respect were performed by Himsworth (1933). Since then, Kempner has fed rice diets to insulin-requiring diabetics with a large measure of success (Kempner et al. 1958), and these contained over 90% of all energy in carbohydrate form. Despite the considerable efforts of these and other workers, carbohydrate restriction has remained standard diabetic dietary practice, perhaps because it seemed logical to limit carbohydrate intake if a patient was deficient in insulin. Even the resurgence of interest in diabetic dietary management over the last 10 years has concentrated rather M y on the indisputable benefits of fibre as opposed to digestible carbohydrate. Some studies do, however, suggest that the proportion of energy from carbohydrate can have an effect on glucose tolerance separate from that of fibre.

A comparison of isocaloric high carbohydrate and high fat test meals in diabetics.
Ten diabetic patients were given test meals at breakfast and lunch on successive days to compare the acute glycaemic responses to meals either high (65%) (HC) or extremely low (12%) in carbohydrate; the latter was very high (71%) in fat (HF). The meals were isocaloric and fibre content, though higher in HC, was within the range of the average British intake for both diets. The mean fasting bloo...

PURPOSE OF REVIEW Recent randomized controlled trials examining diets of varying carbohydrate composition recommended for people with diabetes and cardiovascular disease and those at risk are summarized. RECENT FINDINGS Severe carbohydrate restriction results in appreciable initial weight loss and improvement in risk factors. After a year, however, the beneficial effects are equal to or less ...
متن کاملHigh-fat diets: healthy or unhealthy?
In the current dietary recommendations for the treatment and prevention of Type 2 diabetes and its related complications, there is flexibility in the proportion of energy derived from monounsaturated fat and carbohydrate as a replacement for saturated fat. Over the last few years, several population studies have shown that subjects eating a lot of refined grains and processed foods have a much ...
متن کاملHigh-carbohydrate diets and insulin-dependent diabetics.
A high-carbohydrate-(HC)-modified fat diet was compared with a standard low-carbohydrate (LC) diabetic diet in 11 insulin-dependent diabetics. Basal and preprandial plasma glucose concentrations were appreciably lower when the patients received the HC diet derived chiefly from readily available cereal and vegetable sources (mean (+/- SE of mean) basal concentrations 6.7 +/- 1.2 mmol/l (121 +/- ...
متن کاملA randomised controlled trial to investigate the effect of a high fibre diet on blood pressure and plasma fibrinogen.
Two hundred and one subjects (147 men and 54 women) were randomly allocated to either a high cereal fibre diet or a low cereal fibre diet for four weeks. Each group then followed the alternative diet for a further four weeks. Cereal fibre intakes were 19g/d (31 g/d 21 g/d total fibre) and 6g/d (19g/d total fibre) on the high and low fibre diets respectively (p less than 0.001). Energy, protein,...
